How to file with UnitedHealthcare

  1. Get the UnitedHealthcare claim form

    Download the UnitedHealthcare claim form (PDF) and fill in the member section. Some plans also take claims online after you sign in at myuhc.com.

  2. Gather your receipt and prescription

How to file with Kaiser Permanente

Most Kaiser Permanente plans are HMOs, which usually cover equipment only when it comes through Kaiser. Call Member Services before you order and ask whether your plan pays members back for equipment bought elsewhere.

Insurance questions

Do you bill my insurance?

This page is for filing a claim yourself. You order at our regular price and send your insurer its claim form with your receipt and prescription. Your plan then pays you back what it allows. Nothing about your order waits on an approval.

For Florida residents, we also bill a number of plans directly, including Medicare. If yours is on our list of plans, send us your insurance details and we can run your order through your insurance for you.

Which insurance plans can you bill for me?

We bill Medicare and a number of Medicaid and commercial plans directly for Florida residents, and a few plans for members in Massachusetts and New Jersey. See the plans we work with and what to send us. If your plan is not listed, you can still file a claim yourself using the steps on this page.

Will my insurance pay me back?

It depends on your plan. PPO plans with an out-of-network benefit usually pay a share once you have met your deductible. HMO plans usually pay nothing for equipment bought outside their network. The number on the back of your card is the fastest way to find out.

What counts as an itemized receipt?

One that shows each item, its price, the date and who you bought it from. Your order confirmation email has all of that, and so does the order page in your account. If your insurer wants billing codes printed on it, email us your order number and we will send an invoice with the codes added.

Do I need a prescription to file a claim for supplies?

Insurers want one on file, yes. We do not need a prescription to ship cushions, filters or tubing, but your plan treats them as medical equipment. Send the same prescription you used for your machine or mask, or ask your doctor's office for a copy.

Can I use my HSA or FSA and also file with my insurance?

Use one or the other for the same purchase. Money from an HSA or FSA is already tax free, and the IRS does not allow the same expense to be reimbursed twice. Many people use the card for supplies and file an insurance claim for a larger purchase like a machine.

A complete mask is one line on my receipt. How do I list it?

List it under the mask code, for example A7030 for a full face mask or A7034 for a nasal mask. A complete mask includes the headgear, and most plans accept it that way. If your insurer needs the headgear broken out, contact us with your order number and we will help.

How long does reimbursement take?

Most insurers respond within a few weeks with an explanation of benefits. If you have heard nothing after 30 days, call the claims number on your card and ask for the status of your claim.

Can I file for an order I placed months ago?

Often, yes. Many plans accept a member claim for up to a year after the purchase date, so check your plan's filing deadline. Every order you have placed with us stays under Orders in your account, so the receipt is always there.
